Phase Two Consultation


1. Under its Terms of Reference, the Inquiry is required to consider what recommendations should be made to the Secretary of State for Health and the Home Secretary as to how an event such as Victoria's death might be avoided in the future.

2. Paragraph four of the Inquiry's Terms of Reference says:

"To reach conclusions as to the circumstances leading to Victoria Climbié's death and to make recommendations to the Secretary of State for Health and the Home Department as to how such an event may, as far as possible, be avoided in the future."

3. It is probable that a number of recommendations will be made solely on the evidence received in Phase One. Other recommendations may require more detailed consideration, particularly in relation to their wider implications. Phase Two will provide the mechanism for that work.

Suggestions for topics for Phase Two

4. The Inquiry will seek to identify during Phase One, the issues that will require further consideration in Phase Two. This note sets out a provisional framework for Phase Two. The Inquiry would welcome:

Comments on the structure and content of the provisional framework.

Suggestions for topics to be considered in Phase Two, identifying in each case the heading in the attached draft framework under which that topic would fall.

5. In deciding whether to include a topic, the Inquiry will apply the following criteria:

Does the topic fall within the Inquiry's Terms of Reference?

Is the topic feasible and appropriate for the Inquiry to consider?

How likely is it that consideration of the topic will lead to practical and useful recommendations?

Has the Inquiry heard sufficient evidence on the topic during the course of Phase One for it to be able to make recommendations without further consideration of the topic in Phase Two?

Format for submissions

6. It would be very helpful if your submissions could follow the format of the questions set out in the provisional framework. You should aim to make your submissions as concise as possible.

Phase Two hearings and Interested Parties

7. Once the written submissions have been received and analysed, the Inquiry will decide on the format for the Phase Two hearings. Further information on the hearings and whether there will be 'Interested Parties' will be issued at a later date.
